Summary and Qualifications The Manager coordinates the day-to-day
operations of Case Management, Social Services, and clerical
support in order to support the mission, vision and values of
Medical City Plano. POSITION RESPONSIBILITIES: - Manages department
activities to ensure staff have the proper resources and education
to tend to their respective job. - Practices autonomously,
consistent with practice standards-based policy/procedures. -
Provides daily leadership for case/resource management to achieve
maximum deployment and integration. Measures such functions
throughout the organization using the facility improvement process.
- Utilizes data analysis to drive departmental and facility
improvement projects. - Works cooperatively with physicians to
achieve the utilization of the case management plan. - Collaborates
with clinicians and support staff in designing and implementing
processes to comply with regulatory requirements and assist with
TJC survey readiness/preparation. - Acts as a liaison to payers and
post-acute care providers. - Participates in performance
improvement activities of the department and organization. -
Provides leadership for all areas of the Case Manager and Social
Worker roles. - Audits Medicare Important Message from Medicare
documents, avoidable days, documentation standards for
corporate/regulatory agencies. - Effective monitoring and
completion of required post discharge account needs through
applications such as eRequest and HPF. - Promotes effective and
efficient utilization of clinical resources. - Actively provides
and monitors training of all new departmental staff. - Assists the
Director in the development and check-off of on-going staff
competencies and ensures that all staff members are compliant with
annual safety, HIPAA and Code of Conduct requirements; provides
input on staff performance evaluations. - Functions effectively as
a multi-disciplinary team member; exercises independent judgment
within the context of the interdisciplinary patient care team. -
Maintains compliance with applicable laws, regulations and
standards of those planning, regulatory, and accrediting agencies
that affect the institution. - Participates in development and
evaluation of professional practice by participating on unit and/or
hospital committees, special projects and attendance at staff
meetings and town halls. - Incorporates Standards of Behavior and
Rounding into activities, assures results of rounding is shared
with staff through appropriate methods. What qualifications you
will need: Education: - Bachelor’s Degree in Nursing.
Licensure/Certification/Registration: - Texas State RN license or
compact license is accepted, National Specialty Certification
preferred. Experience: - Minimum 3 years Case Management
experience, supervisory experience a plus. Medical City Plano
